Patient Feedback Form

Patient comments, compliments, suggestions and complaints are always welcome through our Patient Feedback System.

Complete a Patient Feedback Form via the button. All submitted forms will be reviewed in a timely manner by the Executive Director, Richard Matens. Depending upon the patient's stated preference, he may call to follow up with the patient, he may meet with them in person, or correspond in writing.

CTHP is here to serve our patients without bias. We cannot improve if we are not aware of what we can do to better serve our patients. Submitting a Patient Feedback Form will not affect the care a patient receives at CTHP in any way.

Patient Feedback Forms are available in the main lobby literature display holders. Reception staff will also be happy to provide you with a form.

The Annual Patient Satisfaction Survey

Help us do better! Every year CTHP conducts an annual Patient Satisfaction Survey to evaluate patient satisfaction with various aspects of care. The results help CTHP develop an action plan to improve care for our patients.

Results of these surveys are significant, so please complete and submit a survey soon. You can fill it out online, download the form, or get a survey next time you are at CTHP and drop it in one of our survey collection boxes on campus in the reception areas and the main lobby.
